Centralized Intake/Resource Broker:
Egyptian Health Department is seeking someone to serve as the single point of contact for families, schools, medical personnel or other professionals to find appropriate services for children and families for the newly federally funded Systems of Care Grant, Project Connect. The Centralized Intake/Resource Broker will perform centralized intakes; gather identifying demographics information and presenting problems.
Applicant must be extremely organized, have good oral and written communication skills, work well with others and must know all the resources in the area. Bachelors Degree in Human Services related field required, Masters Degree in Human Services field preferred.
Candidate must demonstrate cultural and socioeconomic sensitivity to populations served.
A valid Illinois driver’s license and proof of auto insurance is required.
