WIC (Women, Infants, Children)
Services offered: health/diet assessment, Denver Developmental screening on infants and children, hemoglobin (iron) screening, lead screening, immunizations, nutritional counseling (individual and group), breastfeeding promotion and counseling, coupons for a variety of nutrition and infant formulas.
Case Management
Conditions of Eligibility: pregnant women, postpartum women (up to 6 months after delivery), breastfeeding women (up to 12 months after delivery), infants and/or children up to 5 years. Participants must meet USDA income standards. Participants must have a nutritional/health risk (determined during health/diet assessment)
Immunizations
Services offered: infant, childhood and adult immunizations. Childhood immunizations (2 months -18 years old) offered through VFC Program. Call to find out more about the program. A minimal fee is charged for each immunization. Medicaid is accepted.
Family Case Management
During pregnancy, EHD can help pregnant women, mother’s of infants under the age of one, and infants under the age of one
- in assistance with applying for a medical card
- obtaining medical care
- nutritional counseling
- obtaining transportation to doctor’s appointments
- learning more about having a healthy pregnancy
- and genetic counseling
During baby's first year, our program offers help in
- obtaining medical care
- education on infant care
- family planning and parenting skills
- baby assessments (weight, length, and developmental assessments)
- immunizations
- nutritional counseling
- referrals for WIC
- referrals for 0-3 programs
- visits to assess a high-risk newborn.
Family Planning
Family Planning has been an integral part of Public Health Nursing since September, 2002. This program promotes responsible behavior and the well being of families. It helps to prevent unintended pregnancies through education including abstinence, natural family planning and contraceptive services. It also allows planning and timing of pregnancies. Program funding includes a grant from the U.S. Department of Health and Human Services.
HIV/AIDS Prevention Education
Prevention Education includes presentations provided to groups on a variety of topics relating to HIV, which may contain condom distribution, statistics, brochures and pamphlets, teaching curriculums, videos, and games. Case Management includes healthcare, support groups and counseling, nutrition services, dental care, transportation, housing assistance, utility assistance and client advocacy. To be eligible one must be a resident of the 15 county area of which EHD is a part, with verified HIV+ status, or a family member/caretaker. Availability of some services depends on the individual's income level and/or medical insurance status. There are no fees for services and confidentiality is strictly respected.
STD/HIV Counseling and Testing is available to anyone. There is no charge for these services.
Home Health
A Skilled Nursing Care program offers home visits made by RN’s LPN’s, and Licensed Social Workers, under a physician’s direction. We also offer teaching and supportive care for families and patients with various illnesses. Physical therapy, occupational therapy, speech therapy and psychiatric nursing services arealso available.
